What is CCS?Comprehensive Community Services (CCS) can help people who need ongoing, comprehensive services to help them live, work and participate in their community and ultimately reach a level of recovery in their illness.

CCS Services will be provided in the community, will be flexible and will help the person achieve their vision of recovery.

CCS Services May Include:

  • Service Facilitation and/or Case Management Services
  • Community Living Skills Training/ Daily Living Skills Training
  • Counseling
  • Health Monitoring
  • Employment Related Skills Development
  • AODA Services
  • Crisis Services/Interventions
  • Residential Support
  • Symptom Management
  • Recovery Education
  • Illness Education and Management
  • Diagnostic Evaluations and Specialized Assessments
  • Communication and Interpersonal Skills Training

Each person's support plan will reflect that individual's needs, capabilities, talents and resiliency.

Support in recovery is accomplished best in a natural community setting.

Recovery is non-linear.

Consumers and providers together will develop a recovery plan based on common goals.

Each person's beliefs, values, customs, and culture will be acknowledged and respected.

Consumer's unique needs are the primary focus of all services.
Consumer plays active role in planning and implementing services that are designed to meet their needs.

Builds on Natural & Community Supports
Consumer identifies important individuals to assist with their recovery.

Strength Based
Consumer is assisted in identifying strengths and abilities to aid in their recovery and improve their quality of life.

Self-Sufficiency
Consumer is assisted with developing successful independent livings skills like employment, housing, education, transportation and social recreation.

Goal & Outcome Oriented
Consumer takes a key role in assessment and service planning with goals and outcomes developed to meet the consumer's needs.

Collaborative
Consumer and team members work together to facilitate change in a way that fosters hope, empowerment and recovery.

Cultural Competency
Consumers unique beliefs, values, customs, and culture will be acknowledged, respected and accepted.

Who is Eligible for CCS?

Comprehensive Community Services is available to children and adults who meet the following criteria:

  • Resident of Clark or Trempealeau County;
  • Eligible for Medical Assistance, BadgerCare or another Medicaid Program;
  • Have a mental health and/or substance use diagnosis; and
  • Are determined to need psychosocial rehabilitation services.

How to Apply

  • Individuals, family members or professional staff may refer to the CCS Program
  • Referral Form can be requested from Clark County Community Services or Trempealeau County Human Services
  • Referral will be reviewed by CCS Staff to determine appropriateness for the program
  • Eligibility Screening will be conducted
