Passport applications are accepted by appointment Monday - Friday.

The following items are needed to apply for a passport:
- Certified copy of your birth certificate
- One passport photograph
- Passport fee- check or money order made payable to: US Department of State
- Execution fee- check or money order made payable to: Clerk of Court
- Proof of Identity (valid driver's license, picture I.D., etc.)
